You get conflicts. Every physician has their own philosophical approach to the practice of medicine. Unfortunately, the way that health care is going, physi- cians are being dictated to. Q: Through programs like DRG's? A: Yes. They are being told, "This is the way that you have to do it," particu- larly by Medicare. If we get a patient in here that requires a length of stay that is more than the DRG, they are say- ing that in order to keep the patient any longer we're go- ing to run into a penalty sit- uation. Still, the knowledge of the physician will prevail to the extent that patient really isn't ready to leave this hospital, even if they're receiving pressure from Medicare or an insurance company. Also, obviously they get it (pressure) from the hospital. Q: Physicians may have the final say on length of stay, but the patient is the one who decides whether or not to go to the hospital. Do you find that people who are fully insured may go to the hospital with minor ailments that might not require a hospital visit simply because they know that insurance will Pay for it? A: Yes, you're right. Q: That drives up costs? A: It does. Tl~e more you get into this--most insurance companies will pay for emer- gency room treatment, but they have their own defini- tion of what's an emergency. On the other side of the coin, you run into situations where people use the emer- gency room for a doctor's office visit, which is ex- tremely expensive. The emergency room operates at a loss. There is no way you can receive enough revenue to compensate for the cost of operating an emergency room. Nationwide, a number of hospitals have completely closed their emergency rooms, they don't even offer the service anymore. Q: What about special fa- cilities that deal strictly in emergency care? A: This is a new phenom- enon, too. This is a very pop- ular thing in Metropolitan areas. They price their ser- vices between the hospital emergency room and a dec- tot's office visit. There's a market for it, no question about it. There isn't enough population to have that type of thing here. EUREKA SALE Toot-Pak ~_ with detachable dust cover.
